Domestic Liability Releases & Waivers

These forms are of use to individuals involved with college-associated trips within the United States. All participants must complete a Waiver form and the Emergency Response Form. Trip leaders are required to file an itinerary with the Department of Public Safety.

Module participants should use the waiver below designated for their specific Module.

There are separate forms associated with international travel.

  This form is intended to be filled out by the professor or organizer of any college-sponsored trips and should list all itinerary information.
